Authorization and Consent * By signing this form, parent(s) and/or guardian(s) of potential student(s) understand that a $350.00 non-refundable application fee and a completed Request for Transcript are required as part of the application for admission to a SailAway® Microschool. This fee does not guarantee admission. The fee covers an initial, brief academic and developmental assessment battery. The results of the assessment battery, a review of the student’s application, and interviews with both student and parent(s)/guardian(s) by the SailAway staff and faculty are required before an invitation to enroll is issued. Once admitted, all students receive a full battery of diagnostic and academic assessments. Results of the full battery determine a student’s individual education plan and are reviewed with parent(s)/guardian(s). Should the applicant meet enrollment criteria and space is not available, the student will be placed on a waiting list and you will be notified when an opening becomes available.
